The attached form is a legal document and is good for a period of one year (through August 2009).  Parents of children in kindergarten through 6th grade will no longer have to fill out a form each time their child goes on an event (some exceptions may apply). This document will be used if emergency medical attention is needed.  Parents will always be contacted but in the event a parent cannot be reached, this document will authorize Eagle Heights Church to release medical care.  Email questions to info@crosswalkkids.net
